New Player Instructions

 

Welcome to TUFC!
 
The following is important information for the 2008 spring soccer season. 
 
Please provide me with items 1-4.  All forms can be found on the TUFC web site, www.TUFC.org.  .  
 
1. Medical Release:   The Medical Release form must be notarized.
2. Player and Parent Contract 
3. US Club Soccer Registration Form
4. Birth Certificate: I will need a copy of your child’s birth certificate in order to apply for the US Club and FYSA player passes.
 
5. Photos: I will be taking a photo of your child at practice for her player passes.
 
6. Player Development Fee:  Information regarding this $225 fee will come in a separate e-mail from Warner Sports Promotions.
 
7. Uniform order form and check for $115: Uniform order form is available under "Forms" on the website. You also have the option of ordering a backpack & Adidas Calico Training warm up. Place your order form(s) & check made out to TUFC in the TUFC mailbox at the Meadows. Mark your envelope for Eric Lipman. Or mail to the TUFC mailbox listed under #8 below.
 
8. Player Assessment Application & registration fee: Please complete this form and pay the $125 registration fee.  Place in an envelope marked for the TUFC treasurer. This pays for your player’s, coach’s and manager’s registration with Florida Youth Soccer Association (FYSA) and US Club Soccer, allowing our teams to play in tournaments. This also provides insurance coverage for injuries that may occur during participation.  Please make your check out to TUFC and place in the TUFC mailbox at the Meadows in an envelope marked for the TUFC Treasurer, Garland Sandel.  Put your child’s name and team on the memo line! Be sure to call 893-8989 before traveling to the Meadows to be sure the office is open) or mail to:

9. Team Seasonal Dues: The Spring season fee is $300.  This fee pays for field rentals, coaches, referees and tournament registration fees. This fee is given to Cheryl Everett, our Academy Treasurer and U9 parent.  Parents can either pay in full on Feb. 1 or make four payments of $75 each due Feb. 1, Mar. 1, Apr. 1, and May 1.

10. Practices: Practices are Monday and Thursdays at 5:30-7:00 pm. There is also an optional striker/goalkeeper practice on Wednesday nights at 5:30-7:00 for 6 consecutive weeks beginning February 6th. This optional practice is one of the benefits made possible by the Player Development Fee.
