The following is important information for the 2008 spring soccer season.
Please provide me with items 1-6. All forms can be found on the TUFC website, www.TUFC.org under the Forms link on the home page.
1. Medical Release: The Medical Release form must be notarized.
2. Player and Parent Contract
3. US Club Soccer Registration Form
4. Birth Certificate: I will need a copy of your child’s birth certificate in order to apply for the US Club Soccer Card.
5. TUFC Player Assessment Application
6. Adidas Uniform Order – And optional equipment order form
Photos
I will be taking a photo of your child at practice for his US Club Soccer Player’s Pass.
Fees
Player Development Fee: This $400 fee is payable to Warner Soccer and should be mailed to the address below. Parents may either pay in full on August 1, or make three installments as follows:
August 1st - $150
September 1st - $125
October 1st – 125

Registration fee
This $125 fee pays for the players registration with Florida Youth Soccer Association (FYSA) and US Club Soccer, allowing our teams to play in affiliated tournaments. Please make your check payable to TUFC, mark “Registration Fees” in the memo of the check, and give it to the Team Manager.
Season Fees
The Spring season fee is $200. This fee pays for field rentals, coaches, referees and tournaments. Please make your check payable to TUFC. This fee is given to Cheryl Everett, our Academy Treasurer.. Parents can either pay in full on Feb. 1 or make four payments of $50 each due Feb. 1, Mar. 1, Apr. 1, and May 1.

Uniforms
Required uniforms total up to $115. You also have the option of ordering a backpack & Adidas Calico Training warm up. Give your order form(s) & check made out to TUFC to the Team Manager.
Be sure to put your player’s name and “U10” on all checks.
Practices
Practices will resume on February 4th on Mondays and Thursdays at 5:30-7:00 pm. There is also an optional striker/goalkeeper practice on Wednesday nights at 5:30-7:00 for 6 consecutive weeks beginning February 6th. This optional practice is one of the benefits made possible by the Player Development Fee.
The spring schedule is posted on the web site on the U10 pages, as well as all anticipated tournament events. We are expected to participate in all SSAA playdates marked in red.
