Welcome to the TUFC 2008-09 Season!
The following is important paperwork & fee information for this soccer season.
1. Medical Release: The Medical Release form must be notarized.
2. Player and Parent Contract
3. Internet & Media permission Form
4. US Club Soccer Registration Form (new players or if your information has changed)
5. Birth Certificate: I will need a copy of your child’s birth certificate in order to apply for the
US Club Soccer Card (new players only).
6. Uniform order (more information regarding uniforms below).
7. Photos: I will be taking a photo of your child at practice for his US Club Soccer Player’s Pass.
8. Player Development Fee: This $450 fee is payable to Warner Soccer and should be mailed to:

Please contact Cyndi Boswell (386-3866) if you would like to pay using a credit card. If paying by check, be sure to put your child’s name, U11 & PDF (Player Developmental Fee) on your check.
9. Uniform order form and check for $115: The uniform order form is also on the TUFC website. All new players must order the $115 uniform kit which includes 2 jerseys, 2 pairs of shorts & 2 pairs of socks. You have the option of ordering a backpack & Adidas Calico Training warm up using a separate order form. Your order can not be placed until it is accompanied by a check for the entire purchase payable to TUFC. If you have a returning player and he has his entire uniform from last year, you do not need to make a purchase at this time. If you would like to make any purchase other than the $115 uniform kit, please add %5 to your total bill for shipping.
10. Tryout Request form & registration fee: If you did not complete this form and make the registration fee payment at or prior to tryouts, please complete this form and pay the $125 registration fee. Place in an envelope marked for the TUFC registrar. This pays for your player’s, coach’s and manager’s registration with Florida Youth Soccer Association (FYSA) and US Club Soccer, allowing our teams to play in tournaments. Please make your check payable to TUFC and place in the TUFC mailbox at the Meadows in an envelope marked for the TUFC registrar, Eric Lipman. Be sure to call 893-8989 before traveling to the Meadows to be sure the office is open or mail to:

11. Academy Dues: The fall Academy fee is $300 payable to TUFC. This fee pays for field rentals, coaches, referees and tournaments. It can be paid in full or in increments of $75 over four payments.
